Have you checked how Sibelius fonts (Opus etc.) handle this problem? If I remember correctly, Sibelius has some unusual solutions for standard font problems (e.g. within the Opus Chords font). Maybe it is enough to replicate the way symbols are positioned within Opus Chords etc.

Not being able to examine the score 'in vivo', and with a low resolution picture, I don't think it is done with computer. If you look at the microscopic level how the clefs are inconsistently positioned toward the left line, position of 8va on the clefs, and the time signatures different heights on...
