Most of us use ForScore, which allows us to make extensive annotations (including dynamic, bowing, etc. in addition to text, drawing).

I just started with a production last night and all of the orchestra are using iPads for the music.

One plus is that the copies are coming with adequate time for manual page turns, so most of us aren't even using foot pedals for page turns.

I would start with dashed ties and then throw in a sim. if it's used a lot in the piece.

You can also put in an explanatory note in the score/affected parts.

I tend to stack them vertically.

Absolutely, it is a fine line.

It is instantly understood, however.

I've always (since my teens in the 1950s) labelled instrument changes twice. Initially at the first available rest with ‘take Cor Anglais’ (or whatever) and then again (without ‘take’) immediately before or at the first playing of the new instrument. I can't remember which orchestral player advised...

If you're worried, you can add a sharp in parentheses below the lower note's stem.

K. Stockhausen has something like 93.6 markings, I believe he wants to get EXACT tempo transition (ratio). For me that's a reminder of the sad times when composers tried to be scientists.
