I’m no expert by any means, but in all my time of scanning through music, I feel like this falls on the omission side. Not only is the Treble clef hard to draw with Bezier curves, it's the hardest, period. Keep practicing. You'll get better. I've developed a few strategies over the years, which remain pretty consistent across styles. The one thing my "strategies" can't do for you, though, is give you a better "eye" ...

This is probably the two OS/2 metrics called “Win Ascent” and “Win Descent”. If these numbers are too small, you will see clipping like this, regardless of point size. This doesn’t seem to be an issue outside of Windows.

No requirement. There's hardly anything written about music typography in the first place. You are right, and it's a bit of a trick to make the cautionary treble clef look like it's in the right place, but still be aesthetically pleasing to the eye, when technically it should have the same crosshair look around the second staff line.
