My solution would be to write pppp cresc. p under the repeated bar (or the same with a hairpin), and then in a footnote:
"gradual crescendo through repetitions"

That is "bow vibrato" (i.e. making the rhythm heard by bow pressure only, without stopping the bow between notes).
Sometimes this notation is modernized into a slur with dots on each note. I'm not sure whether that means exactly the same, though.

We're not talking about the same thing. I was talking about this distance

What strikes me in the Malediction examples is the use of the whole-measure rest for all multi-measure rests, instead of the standard multi-measure rest symbol. A short-lived Arnstein experiment, I guess?
